Screening Information

Student and Screener

Do you have a child between the ages of 3 - 5 years old?

Early Childhood Screening allows you to check your child's overall development in a 1 hour appointment.  Screening identifies at an early age possible health or developmental concerns so that your child can get the help they may need before starting kindergarten.  The state of Minnesota now recommends that all children be screened before their fourth birthday.  In order for your child to start kindergarten in any Minnesota public school, you will need to provide your child's Early Childhood Screenig records and immunization record.

Where do I go for screening?

Screening appointments are available in Buffalo, Hanover and Montrose throughout the school year.  Call 763.682.8408 to make your appointment.

Screening forms will be mailed to you after an appointment has been scheduled.  Misplaced the forms?  Just "click" on the form below to print and fill out to bring to your scheduled screening appointment.

Please note:  If your employer is reluctant to allow time off for screening, a 1987 law allows all employees to take up to 16 hours of unpaid leave to attend the educational activities of their children.
